After consultation with legal counsel, the Behavioral Sciences Regulatory Board has authorized the BSRB Executive Director, Max L. Foster, Jr. to inform you that all licenses must be issued in your legal name. Therefore to have your name changed on your license and in the database, you must provide a copy of your driver’s license, passport, military ID or court papers that list your new legal name.
